On your first visit, you'll be asked to complete an information form.
(We hope to have these forms available on our website in a PDF [Adobe
Acrobat] format for quick and easy download and print out very soon in
hopes that this will make your visit to Tahoe Fracture quicker and more
enjoyable) Your responses will aid the doctor in making a diagnosis. Of
course, all information is considered confidential and will not be shared,
unless authorized in writing by you.

Our office hours are from 8:30 am to 5:30 pm, Monday through Friday. Of
necessity, the doctors must work by appointment and patients are seen in
the order in which they have been scheduled. Unfortunately, emergencies do
arise, making some delays and rescheduling inevitable. If your doctor is
unable to keep your appointment, the receptionist will make every effort
to notify you and reschedule your visit by phone.
At the close of an office visit, stop by our reception desk to settle
your bill (See Our
Financial Policy). We recommend scheduling your next appointment at
the same time. You may also make appointments by phone through our
appointment secretary.

If you must cancel an appointment, please do so at
least 24 hours in advance so that we may offer that time to another
patient.
The doctor's clinical notes, as well as any x-rays taken in our
offices, are a part of your medical records and belong to the doctor. We
will gladly send your records to another physician for examination.
Requests should be made, in writing, to our Medical Records Department.
All original x-rays must be returned, however, copies of x-rays can be
made for a fee. A clerical fee will also be charged when clinical notes
are sent to someone other than a doctor.
Please feel free to ask questions about your treatment. It is important
to all of us that you are well-informed and confident. Your treatment may
begin in the office, i.e., splinting, physical therapy or an injection of
a local anesthetic or
medication to reduce inflammation or prevent infection. If surgery is
recommended, it will be scheduled at a hospital in the area. If you must
cancel, or if you develop a cold, cough, sore throat or fever prior to
surgery, please notify your doctor immediately.
