Our financial policy is designed to allow us to provide
quality care to our valued patients. We are always happy to clarify any
point or answer any questions you may have. The financial responsibility
for services rendered rests with you, the patient, or your family,
regardless of your agreement with your insurance company.
We require payment for office visits at the time of service. For our
patients' conveinence, we accept VISA and MasterCard. Please be prepared
for a minimum bill for the first examination of $100.
All charges for services incurred outside the office become due and
payable 90 days from the date of service. This allows sufficient time to
process insurance and to make payment in full of any balance remaining
applicable to the deductible or co-insurance amount.
The following are excluded from the two policies above: patients whose
primary health insurance carrier is Medicare, a health maintenance
organization or preferred provider organization, or who are being treated
for an industrial claim.
All co-payment amounts are due and payable at the time of service, in
accordance with the legal requirements for collecting patient
responsibility amounts.
Insurance forms for services rendered will be completed by our office.
Charges submitted to the insurance carrier will become due and payable 90
days from the date of service. In the event of duplicate payment by the
insurance carrier and the patient, the overpayment will be refunded to the
patient immediately.
We are "Participating Physicians" for Medicare patients. On all
services covered by Medicare, we accept assignment. Medicare will send a
check to our office for 80% of the approved amount. As a service, we will
file supplemental insurances. Although we accept assignment for Medicare
patients, the patient, by law, is responsible for any portion of the
approved amount not covered by Medicare or a secondary carrier.
We are happy to provide treatment for work-related injuries (Workman's
Comp.). Charges incurred are the responsibility of the patient, pending
carrier approval.
Payment for services rendered to dependent children of divorced parents
is the responsiblity of the parent who is seeking treatment, regardless of
court-ordered responsiblity.
We will accept patients covered by benefits from the State Department
of Human Services (ADC, Medicaid, etc.) through local emergency room or
physician referral only. Payment at the time of service is not required,
however, patients are responsibile for the payment of any non-covered
services they request.
